If any physical changes are planned at the address on the application, please submit two sets of plans. One to be 8 ½ x 11, if possible.

If applicant is a corporation, please submit a copy of the Articles of Organization and fill out the attached Vote of the Board of Directors.

Applicant will be required to submit an Interview form for every person listed on the application and return it with three (3) letters of reference.

The license fee is $225.00 which is paid after the granting of the license.

All fees must be paid and the license picked up at the Selectmen's Office before opening for business.
