| COIN-OPERATED AMUSEMENT DEVICES FEE:$175 Per Machine, Per Year Check should be made payable to the town of brookline The applicant must provide the name of the manufacturer, the trade name of the machines, their state i.d. numbers, the name of the distributor, and the agent's name. Three sets of plans should be submitted with the application. The plan shall designate the street and number, entrances and exits, the type of establishment, and the exact location and number of machines to be licensed. A public hearing before the board of selectmen will be scheduled when reports from building, police and human relations/youth resources have been received.
