If any physical changes are planned at the address on the application or if new, please
submit two sets of plans. One to be 81/2 x 11, if possible.
If the applicant is a corporation, please submit a copy of the Articles of Organization
and a vote of the Board of Directors authorizing the license application.
Applicant will be required to submit an Interview form for every person listed on the
application and return it with three (3) letters of reference each.
The license fee is $225.00 which is paid after the granting of the license.
All fees must be paid and the license picked up at the Selectmen's Office before opening
for business.
